As they introduced themselves, a woman collapses nearby; Dankichi's passenger, a foreigner known as Dr. Elder, begins to treat her. They were confronted by Deian Ishizu, a greedy doctor who wants to monopolize Western medicine and despises Dr. Elder's treatment of the poor.
